Sucralfate is an effective anti-ulcer medication. It is a basic aluminum salt of a sulfated sucrose. It is not an antacid (it does not neutralize stomach acid directly) but rather a cytoprotective agent. Its primary action is to form a protective barrier over ulcers in the stomach or duodenum, shielding them from the damaging effects of gastric acid, pepsin, and bile salts, thereby allowing the ulcer to heal naturally.

II. Detailed Mechanism of Action

Forms a Protective Barrier: In the acidic environment of the stomach, sucralfate becomes viscous and cross-links with positively charged proteins (like albumin and fibrinogen) in the ulcer crater to form a stable, insoluble adherent complex. This acts like a “bandage” over the ulcer site.
Adsorbs Pepsin and Bile Acids: It directly adsorbs pepsin and bile acids, inhibiting their damaging and proteolytic effects on the mucosa.
Stimulates Endogenous Defenses：(1)It stimulates the synthesis and release of prostaglandin E2 from the gastric mucosa, increasing the secretion of mucus and bicarbonate, thereby enhancing the mucosa’s natural defense mechanisms.(2)It concentrates epidermal growth factor at the ulcer site, promoting cell proliferation and regeneration.
