QS-21 is an immune-stimulatory saponin that can be used as an effective vaccine adjuvant. QS-21 enhances vaccine immune responses by activating dendritic cells and helper T cells, promoting antibody production and cell-mediated immune responses.
QS-21 is an activator of the NLRP3 inflammasome and leads to subsequent release of caspase-1-dependent pro-inflammatory cytokines IL-1β and IL-18.
In vitro, QS-21 (2.5, 5, 25 μM) treatment of HUVEC and PBMC cells for 24 hours significantly reduced cell viability, with this cytotoxic effect being more pronounced in HUVEC cells.
QS-21 (0-50 μM) treatment of two human gastric cancer cell lines (SNU 1 and KATO III) and a normal human cell line (GES-1) for 24 hours resulted in dose-dependent toxicity in KATO III and SNU 1 cells, but showed almost no cytotoxicity to GES-1 cells.
In vivo, immunization of mice with Aβ42 formulated with QS-21 adjuvant produced the highest antibody titers, followed by CFA/IFA > alum > TMG.
Chemical Structure:
QS-21 is classified as an acylated 3,28-bisdesmodic triterpene glycoside, a complex "saponin" characterized by a hydrophobic triterpene aglycone (quillaic acid) and hydrophilic sugar chains.
QS-21 naturally exists in two isomeric forms—QS-21-Api and QS-21-Xyl—differing only in the terminal sugar (apiose vs. xylose) attached to the fucosyl residue. Both isomers exhibit comparable adjuvant activity.
The molecular complexity of QS-21 is remarkable; its total chemical synthesis requires over 70 steps due to the intricate architecture of the glycosylated triterpene scaffold coupled to the acyl chain.

QS-21 is widely used in preclinical and clinical research as:

The most notable formulation containing QS-21 is GSK's AS01 adjuvant system, which combines QS-21 with monophosphoryl lipid A (MPL) in a liposomal formulation.
AS01 synergistically induces strong antibody and helper T cell responses, enabling dose-sparing effects and enhanced vaccine efficacy.
